Who Must File?

Any person, whether individually or as a partnership, conducting business under any title other than the complete name of the owner(s) or your business is incorporated.

Any corporation doing business in a name other than its corporate name.  The certificate must be completed and filed by a corporate officer.

Where Does One File?

File with the Town Clerk, in every city or town where an office of said business may be situated.

What About a Change?

Upon discontinuing, retiring or withdrawing from such business, or in the case of a change of residence of such person or of the location where the business is conducted, the owner(s) of the business must inform the Town Clerk.

Does the Certificate Have to be Displayed?

No. However, you must provide a copy upon request.

How Much Does it Cost to File?

A Business Certificate Filing is $30.00

Do you need proof of Workers’ Compensation Coverage?

Yes, if you have people working for you.  No business or organization may be issued a license and/or permit, as required by local authority, without providing proof of workers’ compensation coverage ~ MGL, C.152, s25A.  Therefore, you are required to provide the Town Clerk with a Certificate of Insurance showing that you carry Workers’ Compensation Insurance.  You will not be issued your license or permit until the Town Clerk has received this information.  Proof of coverage should be verified by contacting your insurance carrier directly or by calling the Department of Industrial Accidents (DIA), Office of Insurance at 800.323.3249.

What if I do not have any employee’s?

If you feel that you are not required to carry workers’ compensation, you must sign a sworn affidavit that no such coverage is required.  Attached is a copy of the DIA Workers’ Compensation Insurance Affidavit.  A copy of the signed affidavit must be sent to the Town Clerk’s Office.
